Hook and ladder

One of the greatest plays in Dolphin history occurred near the end of the first half of our epic 1981 divisional playoff game with San Diego. This classic "hook-and-ladder" play was a huge momentum-shifter. For 100 points ... name:

(1) The Dolphin QB who threw the pass;

(2) The Wide Receiver who caught the pass; and

(3) The player who took the lateral from (#2) to the endzone.
