I think that`s all of them. I wouldn`t consider Buffalo a dinasty (no titles) but you just said teams that WENT to the SB. the 90`s cowboys are considered a dinasty, but they only went to three SBs
as for the second question, only two teams have gone to three or more straight SBs and those are Miami, 1971-1973 and Buffalo, 1990-1993.

how is buffalo not a dynasty? no other team has done (or in all liklihood will do) what they did.
No doubt is it impressive, but they were never the best and I think that is very important when considering dynasties.
